"MONTHLY GIVEAWAY" TERMS AND CONDITIONS
1. No purchase necessary. Void where prohibited.
2. Register online between midnight, June 1st, 2011 and 11:59pm, PT June 30th, 2011. You may Register for the drawing one time only -- one entry per e-mail/mailing address. This giveaway is open to legal residents of the 50 United States and Washington D.C., 18 years of age and older.
3. Online registration form must be completed in its entirety to be eligible for free gift.
4. One recipient will be selected at random by number generator at www.random.org. from all eligible registrants on or about July 5th, 2011. The odds of obtaining a free gift depend on the number of eligible registrations received. The recipient will be notified by mail and/or e-mail on or about July 5th, 2011. Return of any gift notification as "undeliverable" will result in disqualification and selection of an alternate recipient.
5. One recipient will receive the "Monthly Giveaway" gift, as posted on the entry form or as per discretion of the Fo-Shizzle Design Studios. Approximate retail value up to $50.00.
6. No transfers, substitutions in prizes or cash will be allowed or awarded.
7. The Fo-Shizzle Design Studios reserves the right to modify or terminate this giveaway or disqualify any registrant in the event of technical impairment by computer virus or otherwise. In the event of early termination, Fo-Shizzle Design Studios will randomly select a recipient from all eligible registrations received at the time of termination. In the event of a dispute concerning the identity of the person submitting a registration, it will be deemed to be submitted by the person in whose name the e-mail account is registered.
8. By submitting a registration form, you are agreeing to become a subscriber to our e-News letter.
9. To be eligible for "automatic-entry" into future FDS giveaways, you must remain a current FDS e-News subscriber for a minimum period of three (3) months. Unsubscribing prior to such time will remove your information for all future giveaways.
